If you've ever had a run-in with cavities, you know the drill (no pun intended): After getting a local anesthetic for pain, the dentist removes any decayed dental tissue, as well as some healthy tissue, and then fills the cavity to restore the tooth. It's an effective treatment protocol we've been using for well over a century.
It does, however, have its drawbacks. For one, although necessary, removing healthy dental tissue can weaken the overall tooth structure. The dental drill used during the procedure is also unpleasant to many people: Although it doesn't cause any pain thanks to the anesthetic, the sounds and pressure sensations associated with it can be unsettling.
But advances in dental tools, technology and techniques are addressing these drawbacks in traditional tooth decay treatment. In other words, treating a tooth with cavities today is taking on a lighter touch. Here are 3 reasons why.
Earlier detection. The key to effective treatment is to find tooth decay in its earliest stages. By doing so, we can minimize the damage and reduce the extent of treatment needed. To do this, we're beginning to use advanced diagnostic tools including digital x-rays, intraoral cameras and laser fluorescence to spot decay, often before it's visible to the naked eye.
Re-mineralizing enamel. One of the advantages of early detection is to catch tooth enamel just as it's undergoing loss of its mineral content (demineralization) due to contact with acid. At this stage, a tooth is on the verge of developing a cavity. But we can use minimally invasive measures like topically applied fluoride and CPP-ACP (a milk-based product) that stimulates enamel re-mineralization to prevent cavity formation.
Less invasive treatment. If we do encounter cavities, we no longer need to turn automatically to the dental drill. Air abrasion, the use of fine substance particles under high pressure, can precisely remove decayed material with less loss of healthy tissue than a dental drill. We're also using newer filling materials like composite resins that don't require enlarging cavities as much to accommodate them.
These and other techniques—including laser technology—are providing superior treatment of tooth decay with less invasiveness. They can also make for a more pleasant experience when next you're in the dentist's chair.

If a breath of crisp winter air makes you say, “Ouch!” you're not alone. According to a study published in the Journal of the American Dental Association, one of every eight people suffers from tooth sensitivity. And for those individuals, winter can be a particularly challenging time of year.
Tooth sensitivity can result when the inner part of the tooth, called dentin, is exposed. Dentin is normally protected by enamel above the gum line and cementum below, but if the protective coating is lost, then temperature, pressure and acid from food and drinks can activate the nerves inside the tooth.
If you suffer from tooth sensitivity, these tips may help:
Avoid acidic foods and beverages. It may be common sense to stay away from foods and drinks that are hot or cold enough to make you wince, but also avoid those that are acidic, as acid can erode tooth enamel and increase tooth sensitivity.
Wait an hour to brush your teeth. After consuming acidic food or beverages, give your saliva time to neutralize the acid and strengthen the enamel surface to prevent erosion.
Brush gently. Gums can recede due to over-aggressive brushing, exposing sensitive tooth roots. So brush your teeth gently with a soft-bristled toothbrush and rinse with lukewarm water.
Use toothpaste for sensitive teeth. Toothpaste that is specially formulated for sensitive teeth blocks the pores in the tooth's surface where sensitivity can occur. It may also to help to rub the toothpaste on sensitive areas.
Sometimes, however, sensitive teeth result from dental problems that need professional treatment in the form of an at-home prescription, an in-office treatment like bonding or sealants, or a procedure like a gum graft or root canal. Accordingly, here's the most important tip of all:
Schedule a dental appointment. In an exam, we can look for the cause of your tooth sensitivity so it can be treated properly. Sensitivity may result from receding gums, tooth decay, erosion of the enamel, or other dental problems, such as the following:
- Tooth-grinding. If we detect signs of a nighttime tooth grinding habit that you may not even be aware of, we may recommend a nightguard to wear while sleeping.
- A root infection. If your tooth remains sensitive 30 seconds after eating or drinking something hot or cold, the pulp inside your tooth may be damaged. You may need root canal therapy to remove the infection and stop it from spreading.
- A cracked tooth. A crack in a tooth may not be visible due to its size or location, but a compromised tooth surface can cause sensitivity and could lead to bigger problems if not treated.

Whether performed in a dental office or using a home kit, teeth whitening applications are quite effective for bleaching exterior (extrinsic) stains on enamel surfaces. But what if your discoloration comes from inside the tooth? In this case, extrinsic teeth whitening won’t work — you’ll need to undergo an “internal bleaching” method, which can only be performed in a dentist's office.
There are a number of causes for “intrinsic” staining, including too much fluoride exposure or tetracycline use during childhood. One of the more common causes, though, occurs from root canal treatments used to remove the remnants of the pulp tissue inside a tooth’s pulp chamber and root canals. Certain cements used during the procedure to help seal in the filling material and leftover blood pigments can cause the tooth to darken over time.
To alleviate this discoloration, we use a bleaching agent, usually sodium perborate mixed with a diluted solution of hydrogen peroxide to achieve a safe, accelerated color change. After determining that the root canal filling is still intact and the bone is healthy, we create a small cavity in the back of the tooth to access the pulp chamber. The chamber is cleaned of any debris or stained material and then thoroughly irrigated. The original root canal filling is then sealed off to prevent leakage from the bleaching agent.
We then place the bleaching agent in the cleaned-out space with a cotton pellet and seal it in with a temporary adhesive. This step is repeated for several days until we achieve the desired shade of white. Once that occurs we then seal the dentin with a more permanent filling and then restore the cavity we created with a composite resin bonded to the enamel and dentin.
If we’re successful in achieving the desired color, intrinsic whitening could help you avoid more costly options like veneers or crowns for an otherwise healthy and attractive tooth. The end result would be the same — a beautiful smile without those unsightly stains.
